Responsibilities- Patient Assessments and Care Planning: Conduct thorough patient assessments to identify needs and develop effective care plans in collaboration with the care team.
- Collaboration and Communication: Work closely with the care team to ensure seamless communication and coordination of patient care.
- Patient Education and Support: Provide education and support to patients and their families to promote health and independence.
- Leadership and Supervision: Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure high-quality patient care.
- Documentation and Reporting: Maintain accurate and timely documentation of patient progress and care plans.
- On-Call Responsibilities: Participate in on-call rotations to provide 24/7 support to patients and families.
- Education and Licensure: Current RN license, specific to the state(s) you are assigned to work.
- Experience: One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
- Certifications: Current CPR certification.
- Other: Valid driver's license, reliable transportation, and liability insurance.
