Purpose of the Job:
The Terminal Illness Care Coordinator plays a vital role in ensuring patients receive optimal palliative care services from Remarkable Hospice Companies. As a key member of our interdisciplinary team, you will provide direct care to patients and collaborate with other healthcare professionals to deliver comprehensive care. This is a unique opportunity to utilize your knowledge, skills, and passion for caring for individuals with life-limiting conditions.

Key Functions:
• Assess patient needs through comprehensive evaluations, identifying opportunities for symptom management and quality of life improvement.
• Develop and implement individualized care plans that prioritize patient comfort and well-being.
• Coordinate with multidisciplinary teams to ensure seamless transitions between care settings.
• Educate patients, caregivers, and healthcare providers about end-of-life care options and advance care planning.

Requirements:
* An advanced degree in nursing, either an MSN (Master of Science in Nursing) or DNP (Doctor of Nursing Practice).
* 3 years hospice/palliative experience, preferred.
* Driver's license and proof of current auto liability insurance; no listing in the OIG Excluded Provider listing.
* Certified to practice in the state in which employed.

The estimated salary for this position is $120,000 - $150,000 per year, depending on experience.
