Candidate Profile: We are looking for a compassionate individual who exhibits sensitivity, adaptability, and an inclusive mindset while engaging with patients and families facing serious health challenges. Experience in working alongside community clergy to support hospice clients is crucial. The selected candidate will collaborate with the Hospice Interdisciplinary Team (IDT) to provide spiritual guidance and assist in formulating care plans for patients and their families. Strong teamwork and effective communication within an integrated interdisciplinary team are essential.
Key Responsibilities: As a vital member of our team, your duties will include:
- Coordinating with the Hospice team for patient referrals, conducting home and nursing home visits, and maintaining accurate documentation.
- Communicating clearly with team members while adhering to hospice philosophy and policies.
- Addressing issues related to grief and loss, providing staff consultation, and delivering bereavement follow-up services.
- Promoting high standards of care and customer satisfaction while participating in quality improvement initiatives.
- Enhancing awareness of spiritual needs within the community and liaising with clergy for additional support.
- Planning and officiating funerals or memorial services, as well as participating in memorial events, staff training, and patient conferences.
- Establishing and nurturing a network of clergy for spiritual care resources.

Qualifications:
- Graduate of an accredited seminary or school of theology, or possess relevant certification in hospital or pastoral ministry.
- A minimum of one (1) year of experience as an ordained pastor/rabbi or one (1) year of experience in hospice care.
- Possession of a valid driver's license.
