Candidate Profile: We are looking for a compassionate individual who exhibits sensitivity, adaptability, and an inclusive approach when working with patients and families facing serious illnesses and end-of-life issues. Experience in collaborating with local clergy to assist hospice clients is vital. The selected candidate will participate in the Hospice Interdisciplinary Team (IDT), providing pastoral care and contributing to the development of patient and family care strategies. Strong teamwork and collaboration skills within an integrated interdisciplinary team are essential.
Key Responsibilities: As a valued member of our team, you will:
- Collaborate with the Hospice team for patient referrals, conduct home and nursing home visits, and maintain accurate documentation.
- Communicate effectively with team members while adhering to Hospice principles and policies.
- Address issues related to grief and loss, provide staff consultation, and offer bereavement follow-up services.
- Promote high-quality care and customer satisfaction, actively participating in Quality Improvement initiatives.
- Enhance awareness of spiritual needs within the community and liaise with clergy for support.
- Plan and officiate funerals or memorial services, and engage in Memorial Services, staff training, and patient discussions.
- Establish and maintain a network of clergy for spiritual care resources.

Qualifications:
- Graduate of an accredited seminary or theological institution or hold relevant certification in hospital or pastoral ministry.
- Minimum of one (1) year of experience as an ordained pastor/rabbi or one (1) year of experience in hospice care.
- Possession of a valid driver's license.
