Medical Social Worker III

Kaiser Permanente is seeking a highly skilled Medical Social Worker III to join our team in a medical acute care setting. As a key member of our healthcare team, you will provide assessments, coordination, and implementation/evaluation of professional social services to patients and their families.
Key Responsibilities- Assessment and Intervention: Conduct thorough bio-psychosocial assessments to identify patients' social, emotional, and economic needs, and develop and implement treatment plans to address these needs.
- Case Management: Coordinate with healthcare teams to ensure seamless care transitions and provide education to patients and families on managing chronic conditions and accessing community resources.
- Advocacy and Support: Provide patient and family advocacy services, including crisis intervention, counseling, and support, to promote optimal health outcomes.
- Interdisciplinary Collaboration: Work effectively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and address complex patient needs.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and services, and contribute to the development of policies and procedures to promote best practices.
- Education: Master's degree in Social Work from a school accredited by the Council on Social Work Education.
- License: Licensed Clinical Social Worker (California) with a minimum of one year of experience working in a medical acute care setting.
- Skills: Excellent assessment and intervention skills, strong case management and advocacy skills, ability to work effectively in an interdisciplinary team, and strong communication and interpersonal skills.
