About the Role

We are seeking a highly skilled and compassionate Medical Social Worker to join our team at Snowline Health. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to our patients and their families.

Key Responsibilities
  • Provide comprehensive social work services to patients and their families, including counseling, assessment, and care planning.
  • Collaborate with healthcare teams to develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
  • Conduct thorough assessments of patients' social and emotional responses to illness, caregiving roles, and death and dying issues.
  • Develop and implement effective coping strategies and long-term care plans to support patients and their families.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient interactions, assessments, and care plans in accordance with regulatory standards.
  • Participate in regular team meetings, orientations, trainings, and educational events to stay up-to-date on best practices and industry developments.
Requirements
  • Masters of Social Work Degree from a graduate school of social work accredited by the Council on Social Work Education (Licensed Clinical Social Worker preferred).
  • At least one year of clinical work experience in a healthcare setting or hospice, with a proven track record of providing compassionate and effective social work services.
  • Excellent communication and counseling skills, with the ability to work with diverse populations and belief systems.
  • Demonstrated ability to efficiently operate a computer and prior experience with Electronic Health Records (EHR) is preferred.
  • Valid California driver's license and reliable vehicle insurance.
  • Ability to work occasional flexible schedules, including evenings and weekends, and be comfortable with mobile travel throughout the service area.
