Medical Social Worker

The medical social worker is responsible for the implementation of standards of care for medical social work services.
Essential Job Functions/Responsibilities
- Assist the physician, physician assistant, or advanced registered nurse practitioner and other members of the health team in understanding significant social and emotional factors related to the patient’s health problems; need of care, response to treatment and adjustment to care.
- Assess the social and emotional factors in order to estimate the patient’s capacity and potential to cope with problems of daily living.
- Help the patient and caregiver to understand, accept and follow medical recommendations and provide services planned to restore the patient to optimum social and health adjustment.
- Assist patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care; and
- Identify resources, such as caregivers and community agencies, to assist the patient to resume life in the community, including discharge planning, or to learn to live within his disability.
- Assessment of the relationship of the beneficiary's medical and nursing requirements to the beneficiary's home situation, financial resources and availability of community resources.
- Counseling services that are required by the patient.
- Appropriate action to obtain available community resources to assist in resolving the beneficiary's problem.
- Graduate of a Master’s program in social work accredited by the Council on Social
Work Education.
- May have a doctoral degree in social work.
- Minimum of one (1) year's experience in health care.
- Experience in home health care preferred.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains current CPR certification.
- Computer savvy and own laptop
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
