We are looking for a motivated and proactive **Support Services Coordinator**.

The **Wood County Board of Developmental Disabilities (WCBDD)** is dedicated to facilitating services for individuals with developmental disabilities in our community. Our core mission is to empower, support, and inspire those we serve. The ideal candidate will possess strong problem-solving abilities, be a collaborative team member, and demonstrate effective time management and leadership skills.

The **Support Services Coordinator** will oversee a caseload of clients, responsible for developing individual service plans and ensuring the delivery of services that prioritize health and safety.

Position Details:
  • Classified/Civil Service Appointment
  • Full-Time Position: 80 Bi-Weekly Hours
  • FLSA Status: Non-Exempt
  • Healthcare Benefits: Eligible
  • Standard Working Hours: Monday to Friday, 8:00 AM - 4:30 PM
Key Responsibilities:
  • Organize and lead Individual Service Plan (ISP) and Individual Family Service Plan (IFSP) meetings, utilizing a person-centered approach.
  • Draft and update ISPs/IFSPs, including service authorizations based on assessed needs, in collaboration with the Interdisciplinary Team.
  • Implement, document, and monitor ISP data, ensuring compliance with state and federal regulations.
  • Evaluate, coordinate, and ensure the provision of services aligned with individual needs as consented in the ISP.
  • Guide all service and behavioral programs effectively.
  • Maintain thorough documentation of all programs and service delivery reviews.
  • Facilitate individual and guardian choice in provider selection, assisting as necessary in line with HCBS guidelines.
  • Collaborate with local educational institutions and Early Intervention providers.
  • Act as a liaison among Board components, families, external agencies, and service providers.
  • Assist in securing community-based services for eligible individuals.
  • Provide advocacy for individuals through direct engagement and document all service activities through Targeted Case Management.
  • Offer information and referral services to interested parties, coordinating services and sharing information across agencies.
  • Review and track Unusual Incidents for trends and patterns.
  • Support individuals and families in completing necessary documentation for Waiver enrollment.
  • Coordinate initial and annual evaluations/assessments as required.
  • Maintain ongoing communication with individuals and families regarding service status and ISP updates.
  • Stay informed about HCBS Waivers.
  • Provide technical assistance regarding billing and documentation to individuals and families.
  • Contribute to the development of success plans and monitor their effectiveness.
  • Assist in the completion of forms for review by the Human Rights Committee.
  • Share on-call responsibilities with other coordinators for crisis intervention.
  • Have knowledge of Major Unusual Incidents (MUI) and reporting requirements.

Education and Experience Requirements:
  • Bachelor's degree is required.
  • Preferred fields of study include Social Work, Psychology, Special Education, or related disciplines.
  • A minimum of 2 years of experience working directly with individuals with developmental disabilities is preferred.

Minimum Qualification Standards:

  • Pass pre-employment and random drug and alcohol testing.
  • Receive satisfactory criminal background checks as mandated.
  • Meet physical demands and working conditions as outlined.
