Job Summary:

We are seeking a highly skilled Medical Records Specialist to join our team at Transitional Care Management. As a Medical Records Specialist, you will be responsible for maintaining accurate and up-to-date medical records for our patients. This is an excellent opportunity to work in a dynamic and supportive environment with a highly skilled multidisciplinary team.

Responsibilities:

  • Maintain accurate and up-to-date medical records for patients
  • Ensure compliance with CMS and IDPH rules for the Covid-19 vaccine
  • Work collaboratively with the healthcare team to provide high-quality patient care
  • Participate in ongoing education and training to stay current with industry developments

Requirements:

  • Certified Nursing Assistant (CNA) certification in IL
  • Prior experience in a hospital or short-term rehab unit within a skilled nursing facility is ideal
  • Electronic medical records experience (Matrix) is helpful
