Geriatric Primary Care Provider for Long-Term Care Residents

We are seeking a clinical associate professor to join our team as a geriatric primary care provider for long-term care residents. This is a unique opportunity to work with the University of Wisconsin Madison and contribute to the growth and development of our medical school.
The successful candidate will have a strong background in geriatric medicine, excellent communication skills, and the ability to work effectively in a team environment. We are looking for a dedicated professional who can provide exceptional patient care and make a positive impact on our patients' lives.
About the Role:
- Provide primary care services to patients at skilled nursing facilities
- Teach and supervise medical students and residents in a clinical setting
- Manage a panel of patients at skilled nursing facilities, including chronic disease management and coordination of care
- Collaborate with other healthcare professionals to ensure seamless patient care
Key Responsibilities:
- Provide high-quality patient care and meet minimum work expectations related to patient face-to-face care
- Establish and maintain relationships with skilled nursing facilities and their staff
- Develop and implement individualized care plans for patients, incorporating evidence-based practices and best available research
Qualifications:
- Terminal degree (MD/DO or equivalent)
- Eligibility for a WI medical license
- Board eligibility in general internal medicine or family medicine
Estimated Salary:$160,000-$210,000 per year
