We are seeking a skilled Nursing Care Coordinator - Long-Term Care to join our team at SSM Health. As a Nursing Care Coordinator, you will play a vital role in providing high-quality care to our residents while ensuring compliance with regulatory standards.

Main Responsibilities:
  • Skilled Nursing Care: Administer medications, treatments, and provide nursing services according to established care plans and regulatory requirements.
  • Care Coordination: Coordinate care for patients with diverse health needs, including acute, subacute, and chronically ill residents.
  • Medication Management: Maintain accurate inventory of medications, ensure proper distribution, and monitor for side effects.
  • Leadership and Education: Guide auxiliary staff, participate in report meetings, and provide teaching and training to residents and families as needed.
  • Patient Advocacy: Communicate with residents, guardians, and family members on current conditions and needs, ensuring patient-centered care.
Requirements:
  • Education: Graduate of an accredited school of nursing or equivalent education required.
  • Experience: No experience required; willing to train the right candidate.
