Job Title: Registered Nurse (RN) in Home Health

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Elite Home Health, a part of LHC Group. As a Registered Nurse in Home Health, you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Responsibilities:
  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop the plan of care within state-specific guidelines.
  • Coordinate total plan of care with other healthcare professionals involved in care and help achieve and maintain continuity of patient care.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined.
  • Perform patient comprehensive assessments at designated time points and develop the appropriate plan of care.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
