Job Description
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Bedrock at Home Orlando. In this role, you will provide exceptional home health services to patients in need, working collaboratively with healthcare teams and families to develop personalized care plans and address complex needs.

About the Position
This per diem position requires a flexible and committed individual who can deliver high-quality patient care in a variety of settings. As an LPN, you will be responsible for assessing patient conditions, initiating plan of care, and updating as necessary. Your expertise and compassion will make a tangible difference in the lives of those we serve.

Educational Requirements
We require a current Florida licensure as a Registered Nurse, along with a graduate degree from an accredited school of nursing. Additionally, you must possess a valid state driver's license, reliable transportation, and current automobile insurance. A CPR card and HIV certificate, as well as proof of physical examination, are also required.

Responsibilities
Key responsibilities include developing written care plans, obtaining physician orders, collaborating with healthcare teams, and performing patient care duties with consideration for age-specific needs. You will also supervise Home Health Aides, make referrals to other disciplines or community resources, and maintain accurate documentation.

Compensation
The estimated annual salary for this role is between $65,000 - $90,000, commensurate with experience.
