Job Summary

The Patient Transition Coordinator plays a vital role in ensuring seamless care transitions for patients, promoting the Compassus philosophy, and upholding the Code of Ethical Conduct. This position requires strong organizational skills, attention to detail, and effective communication to build positive working relationships within the company and with external stakeholders.

Key Responsibilities

  • Organize and track patient medical records for accurate transitions to home health providers, confirming start of care for each patient referred.
  • Collaborate with the Care Transition Coordinator to identify post-acute care needs and develop effective transition plans.
  • Work closely with Compassus/Ascension at Home intake teams to ensure smooth transitions.
  • Complete referrals and secure outside agency placement when Compassus/Ascension at Home cannot service the patient.
  • Manage large volumes of information, prioritize tasks, and maintain exceptional customer service standards.
  • Handle confidential and non-routine information, creating general correspondences, memos, and reports as needed.
  • Utilize computer software applications effectively, including word processing, spreadsheets, database, and presentation software.

Requirements

  • Bachelor's degree in a health-related field preferred, with an equivalent combination of education and experience considered.
  • Minimum one year of experience in a related healthcare setting, with home health, hospice, and palliative experience preferred.
  • Prior experience with electronic medical records and knowledge of home health, hospice, and palliative eligibility requirements are advantageous.

Skills and Abilities

  • Strong mathematical skills, including ability to compute rate, ratio, and percentage.
  • Excellent language skills, with ability to read, analyze, and interpret general business periodicals, professional journals, and technical procedures.
  • Strong written and verbal communication skills, with ability to articulate and embrace the hospice philosophy.

Work Environment

The Patient Transition Coordinator works in a fast-paced environment, requiring regular standing, walking, and manual dexterity. Adaptations can be made to accommodate team members with disabilities. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus.
