Haven Healthcare is seeking a skilled and compassionate Clinical Manager to oversee our Home Health Care services in the Ft. Myers and Venice areas. As a key member of our team, you will be responsible for directing, supervising, and evaluating the activities of our clinical staff to ensure exceptional patient care and outcomes.

Key Responsibilities:

  • Evaluate and manage patient care to ensure high-quality outcomes
  • Oversee and manage clinical documentation, including QA and Oasis
  • Lead the implementation of PDGM and QAPI initiatives
  • Participate in DON, Metric, Revenue, and Educational meetings
  • Lead case conferences and staff meetings

Requirements:

  • Graduate of an accredited nursing program
  • Licensed as a LPN in the state of the HHA
  • At least two years of Home Care experience, with one year of supervisory experience preferred
