Job Summary:
We are seeking a skilled and compassionate On-Call Hospice RN to join our team at Metropolitan Jewish Health System. As a key member of our hospice care team, you will provide high-quality nursing care to patients in their homes, ensuring their comfort, dignity, and well-being.
Responsibilities:
- Triage calls for hospice patients from 5pm-8am and provide direct nursing care as needed.
- Develop and update patient care plans, requesting written orders from physicians as necessary.
- Document all nursing care provided and update patient status as needed.
- Communicate changes in patient condition to primary RN case managers.
Requirements:
- Graduate from an accredited school of nursing.
- Minimum 1-2 years experience in medical-surgical, community health, or hospice/oncology nursing.
- Knowledge of Word, Windows, and Excel required.
- Good communication, listening, and teaching skills.
- Valid driver's license and reliable transportation.
