Job Summary

We are seeking a dedicated and compassionate Youth Housing Coordinator to join our team. As a key member of our staff, you will provide professional, high-quality case management and assistance to youth experiencing homelessness within the Minneapolis and neighboring school districts.

Responsibilities
  • Provide case management to youth in alignment with their goals towards sustainable housing and stability.
  • Complete intakes and assessments for new youth participants.
  • Provide youth with referrals to community resources and services.
  • Document all interactions and services provided in our database.
  • Create a safe and supportive environment for all homeless youth.
  • Provide guidance and support to school personnel, Social Workers, and Homeless Liaisons.
  • Maintain safety policies and procedures within outreach locations.
  • Attend and represent Sabathani at various community meetings.
  • Assist with the facilitation of and transportation for weekly support groups.
Requirements
  • Professional or related lived experience in providing case management for at-risk homeless youth ages 16-24 of at least 3 years.
  • Ability to work effectively in a diverse, mission-driven agency committed to the principles of diversity, equity, and inclusion.
  • Establish and maintain effective relationships with youth, families, and community service providers.
  • Clear understanding of effective crisis intervention and programming that is trauma-informed, strengths-based, client-centered, and follows a harm reduction philosophy.
  • Proficiency in MS Word and Excel, and ability to produce letters, emails, and reports of professional quality.
  • Experience using data and working with a database is preferred.
  • Highly responsive with a positive attitude and ability to work cooperatively with different types of people.
  • Ability to communicate in writing and verbally in English; a secondary language is preferred.
