Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at YesCare Health. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Responsibilities
  • Contribute to the assessment of patients' health status and develop effective care plans.
  • Participate in the development and modification of patient care plans, ensuring seamless coordination with healthcare teams.
  • Perform delegated tasks and activities within the scope of your practice, prioritizing patient safety and well-being.
  • Provide direct basic care to assigned patients, ensuring their physical and emotional needs are met.
  • Implement patient education programs, promoting health literacy and empowering patients to take control of their care.
  • Utilize a problem-solving approach to decision-making, ensuring patient-centered care and optimal outcomes.
  • Collaborate with healthcare teams to monitor patient progress, identify areas for improvement, and implement quality improvement initiatives.
  • Participate in patient care committee meetings, contributing to the development of policies and procedures that enhance patient care.
  • Ensure seamless communication with patients, families, and healthcare teams, promoting a culture of transparency and respect.
Requirements
  • Graduate from a Licensed Practical Nursing program.
  • Currently licensed as a Practical Nurse in the state of employment.
  • Active CPR certification.
  • Knowledge of specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies.
  • Background check, security clearance, and drug screening.
