Job Summary

We are seeking a highly skilled and compassionate IDD Program Specialist to join our team at PAHM. As a key member of our support services team, you will play a vital role in ensuring the well-being and success of individuals with intellectual and developmental disabilities.

Key Responsibilities

1. Develop and Implement Individualized Support Plans

Collaborate with individuals, families, and healthcare professionals to create personalized support plans that promote independence, self-advocacy, and community integration.

2. Provide Direct Support and Training

Deliver high-quality direct support services, including training and coaching, to individuals with IDD and their families, promoting skill development and empowerment.

3. Advocate and Coordinate Services

Act as a liaison between individuals, families, and service providers to ensure seamless coordination of services, advocating for the needs and rights of individuals with IDD.

4. Monitor Progress and Evaluate Outcomes

Regularly assess and evaluate the effectiveness of support plans, making adjustments as needed to ensure optimal outcomes and quality of life for individuals with IDD.

5. Collaborate with Interdisciplinary Teams

Work closely with healthcare professionals, social workers, and other stakeholders to ensure comprehensive and coordinated care for individuals with IDD.

6. Stay Current with Best Practices and Regulations

Stay up-to-date with the latest research, best practices, and regulatory requirements in the field of IDD support services, ensuring that services are delivered in accordance with industry standards and guidelines.

7. Document and Report

Maintain accurate and detailed records of support services, progress, and outcomes, reporting as required to stakeholders and regulatory bodies.

8. Perform Other Duties as Assigned

Contribute to the overall success of the organization by performing other duties as assigned by management, including participating in quality improvement initiatives and contributing to the development of policies and procedures.

Requirements

Bachelor's degree in a relevant field, such as special education, psychology, or social work, and at least 3 years of experience working with individuals with IDD. Strong communication, interpersonal, and problem-solving skills are essential for success in this role.
