Job Summary

We are seeking a skilled MDS Coordinator to join our team at Salem Lutheran Homes & Rehab. As a key member of our nursing services division, you will play a vital role in providing high-quality care to our residents.

Responsibilities
  • Conduct thorough assessments of residents' needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure accurate and timely completion of Minimum Data Set (MDS) assessments
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination
  • Maintain accurate and up-to-date records of resident care and progress
  • Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
Requirements
  • RN or LPN licensure in the state of practice
  • Certification as a Resident Assessment Coordinator (RAC-CT) within 6 months of hire
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
