We are seeking a skilled MDS Coordinator to join our care team at the Good Samaritan Society. As a key member of our nursing services team, you will play a vital role in providing high-quality care to our residents.

Key Responsibilities

  • Provide care to assigned residents in a caring, safe, and efficient manner, adhering to location standards, procedures, and individualized resident care plans.
  • Complete the assigned portions of the Minimum Data Set (MDS) for Nursing, ensuring accuracy and validity of the process.
  • Maintain residents' self-esteem, privacy, and confidentiality of personal information.
  • Assist in maintaining safe, pleasant, and orderly resident living areas.
  • Collaborate with other team members to ensure accurate and complete MDS documentation.
  • Communicate effectively with physicians, residents, and families to ensure seamless care coordination.
  • Provide education and support to co-workers, residents, and families on care-related topics.

Requirements

  • Graduate of an accredited school of nursing.
  • RN nursing licensure for the state of practice is required.
  • Certification through the American Association of Nurse Assessment Coordination (AANAC) as a Resident Assessment Coordinator (RAC-CT) within 6 months of hire.
  • Excellent communication and comprehension skills.
  • Knowledge of the RAI/MDS process and basic nursing skills.
