This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans.

Completes the assigned portions of the MDS for Nursing. Maintains residents' self-esteem, privacy and confidentiality of personal information. Assists to maintain safe, pleasant, orderly resident living areas. Completes nursing portion of MDS and reviews for completion and validity of entire process. and provides final quality check prior to MDS transmission. Observes and reports residents' conditions. Directs and assists with co-worker, resident, family/significant other education. Graduate of an accredited school of nursing. RN or LPN nursing licensure for state of practice is required. Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.
