Clinical Care Coordinator

CenterWell Home Health is seeking a dedicated and compassionate Clinical Care Coordinator to join our team. As a key member of our home care services, you will play a vital role in ensuring seamless transitions for patients managing chronic conditions or recovering from illness, injury, or surgery.
Key Responsibilities- Develop and maintain relationships with healthcare providers, hospitals, skilled nursing facilities, and assisted living communities to provide updates on available services.
- Collaborate with clinical teams to determine patient eligibility for our programs.
- Conduct facility visits to assess patient needs and facilitate effective communication among healthcare stakeholders.
- Process physician orders in a timely manner as directed by the program.
- RN/LPN/LVN license.
- Strong knowledge of governmental regulations and Medicare eligibility requirements.
- Excellent customer service, account development capabilities, organization, time management, problem-solving, communication, and selling skills.
- Bachelor's of Science in Nursing preferred.
- A minimum of three years clinical experience preferred.
The estimated annual salary for this position is $62,700 - $83,500 per year, depending on location and individual qualifications.
