• Develop and maintain relationships with healthcare providers and facilities to ensure seamless transitions for patients.
• Collaborate with clinical teams to determine patient eligibility for our programs.
• Conduct facility visits to assess patient needs and facilitate effective communication among healthcare stakeholders.
• Process physician orders in a timely manner as directed by the program.

• RN/LPN/LVN license.
• Strong knowledge of governmental regulations and Medicare eligibility requirements.
• Excellent customer service, account development capabilities, organization, time management, problem-solving, communication, and selling skills.

The estimated annual salary for this position is $61,900 - $82,800 per year, depending on location and individual qualifications.
