About the Role:

Abode Care Partners is seeking a skilled Nurse Practitioner (APRN) to join our team, providing high-quality medical care to seniors and individuals with complex needs in various settings.

The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for working with vulnerable populations.

Key Responsibilities:

  • Deliver comprehensive medical care, including examinations, treatments, and referrals as needed
  • Collaborate with caregivers and other healthcare professionals to maintain a holistic plan of care
  • Order and interpret laboratory and diagnostic testing to inform treatment decisions
  • Ensure accurate and up-to-date patient medical records

Requirements:

  • Active state Nurse Practitioner license
  • Current APRN National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • At least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for daily workflow
  • Proficiency in medical instruments and equipment
  • Demonstrated proficiency in computer-based data management programs and electronic medical record systems
