The Long Term Care Licensed Practical Nurse (LPN) takes on the responsibility and accountability for delivering care to patients in an Assisted Living Facility. This nursing professional demonstrates the ability to make clinical decisions effectively and efficiently under supervision, showcasing critical thinking and performance skills in coordinating patient care. Furthermore, this individual adheres to nursing standards and is accountable for managing patient care while assisting others in their care management duties.

Key Responsibilities:

  • Employs the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living environment.
  • Conducts focused interviews to determine specific patient needs.
  • Evaluates signs and symptoms that indicate physiological and psychological changes in patients.
  • Collects, analyzes, and interprets data from healthcare team members, documenting actual and/or potential nursing diagnoses.
  • Records the patient's care plan utilizing identified nursing diagnoses, expected outcomes, and selected nursing interventions.
  • Identifies and prioritizes actions based on patient care requirements.
  • Monitors alignment between the patient care plan and the actions of medical staff.
  • Executes interventions according to established priorities, care plans, and facility policies.
  • Ensures a safe, comfortable, and therapeutic environment for patients.
  • Initiates and assesses patient and/or family education.
  • Modifies the care plan based on evaluations, changes in medical plans, and the effectiveness of nursing interventions.
  • Documents patient responses to nursing and other interventions.
  • Utilizes clinical judgment in evaluation activities to meet the care needs of an assigned unit, including establishing priorities.
  • Monitors the safety and effectiveness of the environment and equipment.
  • Engages in activities that promote effective interpersonal communication within the facility.
  • Supports professional practice by adhering to: 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare policies.
  • Performs additional duties as assigned or necessary.
Minimum Qualifications:
  • Current Licensed Practical Nurse License for the state of practice.
  • Ability to effectively communicate information to and from appropriate individuals through strong verbal, written, listening, and nonverbal skills.
  • Current Health Certificate (if applicable).
  • Current PPD or Chest X-Ray.
  • One year of prior professional nursing experience preferred.
  • Current BLS certification.
  • Successful completion of screening tests for the clinical specialty areas in which the LPN practices with a score of 80% or greater.
  • Successful completion of Background Screening Process.
