Position Overview

Base Pay: $34.60 / hour

The LVN Access Nurse plays a crucial role in supporting the RN Coordinator and healthcare providers by utilizing the nursing process effectively. This position involves administering medications, including immunizations, through various routes, excluding IV. In triage situations, whether over the phone or in person, the LVN Access Nurse is responsible for collecting essential patient information to facilitate consultations with licensed professionals qualified for independent triage. The LVN Access Nurse is expected to exhibit strong critical thinking abilities, leadership qualities, and serve as a positive role model.

Key Responsibilities
  • Employs nursing process elements for data collection and documentation to aid in assessments, care planning, and implementation.
  • Initiates venipuncture for IV fluid administration when certified.
  • Conducts urinary bladder catheterization as per clinician's orders.
  • Manages controlled substances in accordance with clinic policies.
  • Engages in face-to-face or telephone triage using established protocols, consulting with supervising practitioners as necessary.
  • Accurately transcribes verbal or telephone orders from clinicians.
  • Processes new prescriptions and adjustments based on provider orders.
  • Acts as Team Leader when designated.
  • Handles medication refills following established protocols.
Supporting Clinicians and Daily Operations
  • Prepares patients and assists clinicians during examinations and procedures as required.
  • Administers injections and manages diagnostic tests/referrals as directed, including immunizations and lab requisitions.
  • Conducts age-appropriate screenings as needed or instructed by clinicians.
  • Performs thorough nursing assessments for patients in urgent care and initiates emergency protocols as necessary.
  • Ensures completion of end-of-shift responsibilities.
  • Facilitates efficient patient flow by monitoring clinician schedules and collaborating with staff to reduce wait times.
  • Adheres to and instructs others on Standardized Precautions in compliance with relevant health standards.
  • Schedules patient appointments effectively.
  • Notifies management of required supplies and equipment.
  • Reports safety hazards promptly.
  • Performs additional duties as assigned.
Qualifications
  • Current California LVN license and CPR certification.
  • Minimum of one year’s experience in acute care nursing.
Preferred Skills
  • Experience in an outpatient clinic or medical office setting.
  • Typing speed of 45 WPM or higher.
