We are seeking a skilled Registered Nurse to join our team as a Care Manager. As a Care Manager, you will be responsible for providing high-quality patient care and collaborating with the nursing team to develop effective plans of care.
Key Responsibilities:- Perform comprehensive nursing assessments and develop individualized plans of care.
- Collaborate with the nursing team and other disciplines to ensure seamless care delivery.
- Delegate patient care responsibilities to staff and supervise to ensure tasks are implemented effectively.
- Communicate patient information with assigned staff and between shifts.
- Graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification required.
- Competitive hourly rate: $36 - $40 per hour.
