About the Role

We are seeking a highly skilled Pharmacist to join our team as a Home Infusion Therapist. As a key member of our Pharmacy team, you will be responsible for providing pharmaceutical care to patients in their homes, working closely with physicians, nurses, and other healthcare professionals to ensure the safe and effective delivery of medications.

Responsibilities
  • Evaluate and monitor drug therapy for home care patients, collaborating with healthcare professionals to optimize treatment plans.
  • Adhere to organization policies, state and federal laws, and regulations governing pharmacy practice.
  • Participate in quality assurance activities and adhere to standards of accreditation.
  • Coordinate new referrals and admissions, collaborating with nurses, customer service, and distribution teams.
  • Review, receive, and verify patient orders, documenting on order forms.
  • Assist physicians and healthcare professionals with planning and implementing patient care, setting therapeutic objectives, and establishing monitoring parameters.
  • Conduct and document initial and ongoing assessments of patient, drug, and disease information to identify potential drug-related problems or needs.
  • Monitor drug therapy outcomes, collecting and interpreting laboratory data, clinical data, and patient information.
  • Provide consultation and answer inquiries from patients, families, physicians, and healthcare professionals regarding drugs and their use.
  • Maintain thorough documentation in patient records of clinical activities and communication.
  • Process and check medications, supplies, and equipment prior to dispensing from the pharmacy.
Requirements
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
  • Experience with compounding infused drugs.
  • Proven record of evaluating and monitoring drug therapies for home care patients.
  • Good organizational and time-management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Excellent verbal and written communication skills.
  • Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis.
