About the RoleThe Physical Therapist will be responsible for managing the complete process of therapeutic care for an assigned group of patients, working in coordination with the physician and the patient care team. You will provide or direct a majority of the patient's physical therapy to support patients throughout the continuum of care.
Key Responsibilities- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
- Collaborate with the patient care team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on disease management, self-care, and wellness.
