About the Role

We are seeking a highly skilled Pharmacist to join our team at Amerita, a leading provider of home infusion therapy. As a key member of our Pharmacy team, you will play a critical role in delivering exceptional patient care and contributing to our company's mission to provide high-quality, patient-centered services.

Responsibilities
  • Pharmaceutical Care: Evaluate and monitor drug therapy for home care patients, collaborating with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
  • Regulatory Compliance: Adhere to organization policies, procedures, and applicable state and federal laws and regulations governing pharmacy practice.
  • Quality Assurance: Participate in quality assurance activities and adhere to all appropriate standards of accreditation.
  • Patient Coordination: Coordinate and facilitate new referrals and admissions, collaborating with nurses, customer service, and distribution teams.
  • Order Review: Review, receive, and verify all new patient orders, documenting on the respective order form.
  • Consultation and Education: Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
  • Documentation: Maintain thorough and appropriate documentation in the customer/patient record of all clinical activities and communication.
  • Medication Processing: Process and check all medications, supplies, and equipment prior to dispensing from the pharmacy.
Requirements
  • Education: Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Licensure: Registered Pharmacist License in state(s) of practice.
  • Experience: Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
  • Skills: Proven record of evaluating and monitoring drug therapies for home care patients, good organizational and time-management skills, and excellent verbal and written communication skills.
