The Triage Nurse Specialist is a critical member of the Home Health interdisciplinary team, responsible for delivering exceptional care through triage calls after hours and on holidays. This role requires strong clinical expertise, excellent communication skills, and the ability to prioritize tasks effectively.
Key Responsibilities- Triaging Patient Calls: Respond to patient inquiries and concerns in a timely and efficient manner, ensuring that all tasks are completed within 30 minutes of request.
- Escalation and Problem-Solving: Escalate complex issues to the manager when a resolution is not identified by the user, and participate in department staff meetings and educational sessions to enhance clinical knowledge.
- Task Prioritization and Management: Prioritize incoming tasks according to patient needs, meeting minimum productivity requirements of 100 tasks per day on weekends and holidays, and 50 tasks per day on weekdays.
- Collaboration and Communication: Work closely with the interdisciplinary team to ensure seamless care delivery, and maintain excellent customer service and communication skills.
- Education and Experience: Associate Degree or certification equivalent, and 3 years of experience in a related field.
- Licensure and Certification: Current RN Licensure for all applicable states assigned prior to accepting calls for agencies.
- Skills and Abilities: Proficient in HCHB, strong problem-solving and analytic skills, excellent computer skills, and high-speed internet access.
