Clinical Nurse Specialist

Job Summary:
The Triage Nurse is a vital member of the Hospice interdisciplinary team, responsible for delivering high-quality care through triage calls after hours and on holidays. This role requires excellent communication and problem-solving skills to assess patient needs and provide compassionate care.
Key Responsibilities:
- Receive and respond to patient and family calls, assessing problems and delivering high-quality, patient-focused care.
- Document calls and workflow in the electronic medical records system, ensuring timely and accurate documentation.
- Coordinate with on-call nurses to deliver high-quality nursing care and schedule assessments as needed.
- Maintain effective communication with patients, families, employees, physicians, vendors, and referral sources during after-hours on-call.
- Update staff and the interdisciplinary team on patient changes, vendor, and facility updates to ensure compliance with the patient's plan of care.
- Assist in overseeing medication, DME, and supply utilization to support the organization's financial performance.
- Communicate patient status changes or needs to medical directors, attending physicians, and other stakeholders.
Requirements:
- Licensed Registered Nurse in good standing with no restrictions in all states of practice.
- Two years' experience as a Registered Nurse, with at least one year of direct patient care in a hospice, home health, or oncology setting.
