We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team at SunCrest Home Health, a part of LHC Group.

As a LPN, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.

Responsibilities:

  • Provide quality patient care and perform technical skilled care in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Instruct patients and their families in the care of the patient and maintenance of a healthy environment.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.

Qualifications:

  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
