Job Title: Licensed Practical Nurse

Job Summary:

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team in Winston Salem, NC. As a PRN (as needed) opportunity, you will provide safe patient care, deliver individualized nursing care plans, and work as part of an interdisciplinary team to support patients and their families.

Responsibilities:

* Communicate health/nursing needs and/or recommendations to RN supervisor
* Coordinate admissions, discharges, and follow-up as needed
* Assist in medical/health appointments as needed
* Ensure the provision of nursing/health services under the direction of the RN and as ordered by the physician
* Provide direct care as outlined in the Service Plan/Nursing Care Plan
* Take and record vital signs, weights, wound measurements, etc.
* Identify trends outside of parameters, or in need of attention and communicate to RN/physician

Qualifications:

* Must be a Licensed Practical/Vocational Nurse (LPN or LVN, per state licensure requirements) in good standing
* CPR certification
* Six (6) months of direct nursing experience
