Job Summary:
We are seeking an experienced Administrative Case Manager to join our team. The successful candidate will be responsible for administering case management services to clients, including data entry and note entry on HIMS.
Key Responsibilities:
- Establishing communication with clients over phone, email, and in-person quarterly meetings.
- Providing strong client control, data entry, and note entry on HIMS.
- Assisting not housed individuals who are in need of belonging to a program.
Requirements:
- Strong communication, verbal and written skills.
- Availability to drive within 10 miles once a month.
- Experience with Case Managing or Housing Supportive Services.
