Job Description

  • I. Individualized Service Plan (ISP) Development and Implementation:
    • Participate in the development of the ISP by providing accurate and relevant information about the individual's needs and preferences.
    • Contribute to ongoing assessments and reviews of ISP goals, ensuring they effectively address the individual's needs.
    • Request and assist with addendums to the ISP when necessary, working with stakeholders to implement changes in a timely and coordinated manner.
  • II. Behavior Support Plans:
    • Contribute to the development and implementation of Behavior Support Plans to help individuals address challenging behaviors while promoting positive strategies.
    • Collaborate with the clinical team to ensure that Behavior Support Plans are implemented consistently and evaluated regularly for effectiveness.
  • III. Exit Interviews and Discharge Planning:
    • Complete exit interviews with individuals transitioning out of services, ensuring a comprehensive discharge plan is in place that supports their continued success post-transition.
  • IV. Record Keeping and Documentation:
    • Maintain accurate and organized service records for each individual, ensuring all required documentation is filed promptly according to internal policies and regulatory standards.
    • Ensure that documentation complies with accreditation standards and supports continuous quality improvement.
  • V. Person-Centered Advocacy:
    • Use person-centered tools and approaches to advocate for individual choice and empowerment.
    • Provide guidance and support to individuals in making informed decisions regarding their services, fostering independence and self-advocacy.
    • Offer constructive feedback and advice in helping individuals achieve their personal goals and desired outcomes.
  • VI. Support for Stable Living and Independence:
    • Serve as the primary liaison between individuals and the various services and supports in their lives, including residential staff, family members, service providers, and others.
    • Stay informed about the individual's important to and important for needs to ensure their preferences and practical needs are met.
    • Advocate for individuals' dignity and respect, ensuring they are treated with fairness and kindness.
    • Resolve conflicts or challenges that may arise, supporting individuals to maintain a stable and supportive living environment.
    • Promote independence by identifying opportunities for individuals to increase their autonomy and success.
  • VII. Team Collaboration and Efficiency:
    • Complete tracking notes and documentation promptly and according to department and organizational guidelines.
    • Contribute to the development of team goals and schedules, ensuring both individual and team objectives are met.
    • Utilize organizational tools to manage time and responsibilities effectively.
    • Communicate with team members, coworkers, and external partners in a professional and respectful manner.
    • Demonstrate responsibility by being punctual, prepared, and meeting commitments.
    • Participate in special projects and initiatives as requested by the team or organization.
    • Use organizational resources efficiently and support colleagues by offering assistance when necessary.
  • Other Requirements:
    • Willingness to travel as required, utilizing personal vehicle for job-related travel.
    • Participate in annual performance reviews, providing self-assessment and feedback.
    • Perform additional duties as assigned by the supervisor, as necessary to support the mission of the organization.
    • Completes periodic reviews of the residents to ensure adherence to regulatory guidelines.
    • Addresses civic groups, churches, clubs, and organizations regarding the organization.
    • Coordinates with other human services agencies and utilizes generic services in the community to meet the needs of the people we support.
    • Contributes information to be disseminated in the newsletters regarding the individuals, their success stories, and other related information.
    • Assists with the completion of Personal Outcome Measures interviewing – will need to become certified in the area with CQL.
    • Occasionally fills-in at a group home in a staffing emergency.
