We are seeking a skilled Clinical Therapist to join our team at Open Sky Community Services. As a key member of our Adult Community Clinical Services team, you will play a vital role in delivering evidence-based interventions to individuals with mental health challenges.
Key Responsibilities- Develop and implement comprehensive assessments and individualized treatment plans using person-centered planning.
- Build relationships with individuals served and develop shared understanding of their goals and the steps necessary to achieve them.
- Collaborate with care coordination entities, emergency service providers, outpatient clinics, and other providers to ensure the needs of individuals served are fully met.
- Conduct individual sessions, facilitate support groups, and treatment planning meetings with individuals served.
- Provide clinical consultation and training to staff on the integrated team.
To be successful in this role, you will need a Master's degree in a social services field with current licensure or license eligibility. Previous experience working with adults with mental health challenges, a valid driver's license, and an acceptable driving record are also required.
