Job Summary

We are seeking a skilled Palliative Care Registered Nurse to join our team at Trinity Health At Home. As a Palliative Care RN, you will provide primary nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care.

Key Responsibilities
  • Assess patient condition and develop individualized care plans to improve patient outcomes and satisfaction.
  • Collaborate with physicians, case managers, and other healthcare professionals to coordinate patient care.
  • Provide education and support to patients and their families to promote independence and self-care.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements
  • Graduate of an accredited nursing program with a current RN license in the State of Connecticut.
  • At least one year of experience as a professional acute care nurse, with a preference for home care experience.
  • Current driver's license and reliable transportation.
