About the Role

We are seeking a skilled Case Manager to join our team at Accord, where you will play a vital role in supporting individuals with disabilities in Anoka County. As a Case Manager, you will be responsible for assessing the needs of our clients, developing personalized care plans, and coordinating services to ensure they receive the best possible support.

Key Responsibilities
  • Develop and implement community support plans for each individual, setting goals and objectives to promote independence and well-being.
  • Assist individuals in accessing services, developing service agreements, and documenting progress through case notes.
  • Inform individuals or their legal guardians of service options that align with their needs and preferences.
  • Meet regularly with each individual to evaluate progress, adjust care plans as needed, and ensure they are receiving the necessary support.
  • Identify and engage potential providers to help individuals achieve their goals.
  • Evaluate and monitor the effectiveness of services, ensuring they meet the individual's needs and comply with relevant regulations.
  • Adhere to the case management rule 185 and comply with Minnesota Health Care Programs requirements for waiver services.
Requirements
  • Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field.
  • Current licensure as a Social Worker (if holding a Social Work degree) or at least one year of experience working with individuals with mental illness or disability.
  • Excellent written and oral communication skills.
  • Ability to work independently and in crisis situations.
  • Comfortable with high-volume paperwork and attention to detail.
  • Able to pass a DHS background check, possess a valid driver's license, and maintain a clean driving record.
  • Reliable transportation.
Work Environment

The Case Manager will work primarily in the community and office setting, with some flexibility based on program needs. The role is full-time, non-exempt, and requires 40 hours of work per week.
