**About the Role:**
Avera is seeking a skilled Registered Nurse to serve as a Patient Care Coordinator. This role is responsible for coordinating patient care services between hospital settings and home settings.
**Key Responsibilities:**
- Coordinate patient care services between hospital settings and home settings
- Promote, educate, and coordinate services for patients transitioning from hospital to home healthcare or hospice services
- Collaborate with multidisciplinary teams to ensure seamless patient care
- Develop and implement individualized care plans to meet patient needs
- Provide education and support to patients and families on disease management and self-care
**Requirements:**
- Bachelor's degree in nursing preferred
- Graduate from a CCNE or NLN accredited nursing program strongly preferred
- Minimum 2 years home health/hospice related experience and/or training preferred
- Sound knowledge of community resources preferred
