Avera is seeking a skilled Registered Nurse to serve as a Patient Care Coordinator. This role is responsible for coordinating patient care services between hospital settings and home settings.

**Key Responsibilities:**

  • Coordinate patient care services between hospital settings and home settings
  • Promote, educate, and coordinate services for patients transitioning from hospital to home healthcare or hospice services
  • Collaborate with multidisciplinary teams to ensure seamless patient care
  • Develop and implement individualized care plans to meet patient needs
  • Provide education and support to patients and families on disease management and self-care

**Requirements:**

  • Bachelor's degree in nursing preferred
  • Graduate from a CCNE or NLN accredited nursing program strongly preferred
  • Minimum 2 years home health/hospice related experience and/or training preferred
  • Sound knowledge of community resources preferred
