Job Summary

We are seeking a highly skilled and experienced Medical Staff Coordinator to join our team at Yale-New Haven Health. The successful candidate will be responsible for coordinating and implementing services and activities related to the Diabetes Excellence in Care program.

Key Responsibilities
  • Collaborate with physicians, nursing staff, educators, and allied health professionals to optimize care and clinical outcomes.
  • Develop and implement glycemic management order sets, policies, and standard operating procedures for inpatient diabetes and glycemic management.
  • Participate in program development and quality improvement initiatives to enhance patient care and outcomes.
  • Monitor quality standards and data management of key quality indicators.
  • Coordinate the Joint Commission survey activities and identify areas for improvement.
Requirements
  • Bachelor's degree in nursing or nutrition.
  • At least 2 years of clinical experience in a hospital, outpatient program, or medical practice, including the care and education of patients with diabetes and education of healthcare providers.
  • Current RN license in Connecticut or Registered Dietitian certification by the Commission on Dietetic Registration.
  • Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM) certification preferred.
Working Conditions

The Medical Staff Coordinator will work in a fast-paced environment with moderate stress from meeting schedules, documentation, and regulatory requirements. The successful candidate must be able to read standard type, communicate by telephone/in person, and stand/walk for more than half the day.
