Job Summary

VITAS Healthcare is seeking a compassionate and skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will provide high-quality, patient-centered care to individuals and families facing terminal illness and bereavement.

Key Responsibilities
  • Coordinate care and ensure continuous assessment of patients' and families' needs
  • Develop and implement individualized plans of care in collaboration with the interdisciplinary team
  • Provide education and training to patients, families, and caregivers on palliative care and symptom management
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
  • Represent VITAS Healthcare in a positive and professional manner
Requirements
  • Current licensure to practice nursing in the state where the VITAS program is located
  • A minimum of two years of nursing experience in hospice, home health, or community health
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
