Job Summary:

Hospice Of Hope, Inc is seeking a skilled LPN to provide end-of-life nursing care to individuals and families in the inpatient setting. As a key member of our team, you will work under the direction of the registered nurse to deliver high-quality patient care.

Responsibilities:

  • Provide nursing care to patients in the inpatient setting, adhering to established standards and protocols.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
  • Document patient progress and outcomes, maintaining accurate and detailed records.
  • Communicate effectively with patients, families, and healthcare team members, providing education and support as needed.

Requirements:

  • Graduate of an accredited School of Practical Nursing, current LPN licensure (or eligibility) in Ohio and Kentucky.
  • At least one year of experience in a patient care position in a hospital, nursing home, or related healthcare facility preferred.
  • Personal qualities of empathy, responsibility, and dependability.
  • Effective communication skills with team members, clients, and family members.
  • Ability to arrange travel and access facilities independently.
