The Clinical Pharmacist Specialist is a key member of our healthcare team, responsible for ensuring the safe and effective use of medications in our patient care areas. This role requires a strong foundation in pharmacology, clinical pharmacy practice, and leadership skills.
Key Responsibilities:- Provide expert clinical pharmacy services, including drug therapy management, pharmacokinetics, and multidisciplinary rounding.
- Assist nurses and prescribers with medication use challenges and medication cost containment.
- Develop and implement therapy protocols, manage adverse drug reactions, and evaluate non-formulary requests.
- Collaborate with pharmacy staff and other clinical staff to ensure seamless operational functions.
- Monitor and evaluate the effects of drug therapy, documenting interventions in the electronic medical record.
- Provide education and counseling to patients on appropriate drug use and storage.
- Precept pharmacy residents and students, providing timely feedback on their progress.
- Participate in the Pharmacy and Therapeutics committee, contributing to drug use evaluation programs.
- Stay up-to-date with the latest developments in pharmacology and clinical pharmacy practice, applying this knowledge to improve patient care.
- Doctor of Pharmacy degree or BS pharmacy degree with extensive pediatric experience.
- Current licensure by the Ohio State Board of Pharmacy.
- Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Pediatric Pharmacy Specialist (BCPPS) certification within one year of hire.
- Post Graduate Year 1 Pharmacy Residency (PGY1) required, with PGY2 Residency strongly encouraged.
- 1-2 years of relevant experience, with none in supervisory roles.
