About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Sacred Heart Medical Center in Spokane, WA. As a Clinical Pharmacist - Transitions of Care Specialist, you will play a critical role in ensuring the safe and effective transition of patients from one level of care to another.

Key Responsibilities
  • Lead medication reconciliation efforts for hospitalized patients
  • Collaborate with healthcare teams to optimize patient care and outcomes
  • Develop and implement quality improvement initiatives to enhance patient safety and satisfaction
  • Precept students and residents to promote professional growth and development
Requirements
  • PharmD or Bachelor's Degree in Pharmacy
  • Washington Pharmacist License (or eligible for licensure)
  • Basic Life Support-Provider (BLS) certification (where applicable)
Preferred Qualifications
  • Acute care hospital pharmacy experience (1 year or equivalent)
