RN - PACE/Elderplace Care Manager

We are seeking a skilled and compassionate Registered Nurse (RN) to join our PACE (Program of All-Inclusive Care for the Elderly) team as a Care Manager. In this role, you will provide high-quality care to frail elderly patients with complex medical and psychosocial issues.
Key Responsibilities- Provide care management, care coordination, and skilled care services to at-risk elderly patients.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team (IDT) to ensure comprehensive care and support for patients.
- Assist providers in identifying patient needs and coordinating access to appropriate care and services.
- Monitor and evaluate patient outcomes to ensure quality care and make adjustments to care plans as needed.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
- 1 year of experience in a PACE organization or similar setting.
