Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Potomac Healthcare Solutions. As a Clinical Pharmacist, you will play a critical role in providing comprehensive pharmaceutical services to our patients.

Job Summary:

The Clinical Pharmacist will be responsible for reviewing and verifying medication orders, providing guidance on medication therapy, and educating patients on proper medication use. You will also work closely with clinical staff to develop patient wellness programs and provide medication information and therapy guidelines.

Responsibilities:
  • Review and verify medication orders for accuracy and appropriateness
  • Provide guidance on medication therapy and educate patients on proper medication use
  • Work closely with clinical staff to develop patient wellness programs
  • Provide medication information and therapy guidelines to clinical staff
  • Compound medications as required
  • Ensure prescription directions are accurate and up-to-date
Requirements:
  • Doctor of Pharmacy (Pharm.D.) degree
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE)
  • 12 months of full-time experience within the previous 24 months (Option 1)
  • Board Certification in one or more pharmacy specialties recognized by the Board of Pharmaceutical Specialties (Option 2)
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy (Option 3)
  • Current, full, active, and unrestricted license to practice as a pharmacist
  • Must be a US Citizen
