**Key Responsibilities:**
* Review and verify medication orders for oncology, IV therapy, neonatal care, nutrition, pain therapy, and geriatrics
* Provide guidance on medication dispensing, purchasing, and monitoring
* Assist physicians and clinicians during rounds to optimize patient medication plans
* Educate patients on medication use and management
* Operate electronic medication vaults and pharmacy computer systems
* Deliver comprehensive pharmaceutical services within facility capabilities
* Counsel patients on medication use and provide education on medication management
* Review patient profiles for medication therapy
* Ensure prescription directions are accurate and appropriate for individual patients
* Intervene with providers for discrepancies in medication orders
* Compound medications as required
* Work closely with clinical staff to develop patient wellness programs
* Provide medication information and guidelines to clinical staff
* Provide clinical direction to pharmacy personnel
**Requirements:**
* Doctor of Pharmacy (Ph.D.) degree
* Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE)
* Option 1: 12 months of full-time experience within the previous 24 months
* Option 2: Board Certification in one or more pharmacy specialties recognized by the Board of Pharmaceutical Specialties
* Option 3: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy
* Current, full, active, and unrestricted license to practice as a pharmacist
* Must be a US Citizen
