Job Title: Youth Case Worker

Join our team at the Garden of Prayer Youth Center as a Youth Case Worker, where you will play a vital role in supporting young adults in their transition to adulthood.

Job Summary:

The Youth Case Worker provides individualized services to 6-8 young adults, ages 18-21, in their homes and communities. This includes providing face-to-face support, phone, text, or in-person support as needed, and collaborating with external stakeholders and referral sources.

Key Responsibilities:
  • Provide individualized services to young adults, including case management, counseling, and support.
  • Collaborate with external stakeholders, such as probation, child welfare workers, therapists, and schools.
  • Develop and implement service plans on a monthly basis, based on ongoing assessments of the young person's needs.
  • Document all service contacts and interactions with young adults, families, and other stakeholders.
  • Attend meetings related to the young adult and/or family, and advocate for their best interests.
  • Be on call 24/7, responding in person as needed, and participating in individual and team supervision sessions.
Requirements:
  • Bachelor's degree in a Human Services field.
  • Knowledge of youth development, adolescent issues, and positive youth development/strength-based approach.
  • Must have the Child Welfare Employee Licensure at hire or attend the next scheduled Foundation Training Program for licensure.
  • Valid Illinois Driver's License and proof of insurance.
  • Personal vehicle or access to a vehicle.
Preferred Qualifications:
  • Experience working with youth and families in a case management or counseling role.
  • Knowledge of trauma-informed care and the 5 S's of trauma: Safety, Self-Regulation/Soothing, Supportive, Strengths, and Self-Care.
  • Ability to communicate and work with children with varying skills and ages, and provide necessary guidance, support, and supervision.
